Applicant privacy notice

This notice explains how Nova Physical Therapy collects and uses information submitted through its careers experience. It applies to Nova Physical Therapy recruiting records and employment applications.
What we collect
We collect the contact details, location, role or discipline, professional experience, availability, qualifications, résumé, and messages that you choose to submit. We may collect professional license details when relevant to a clinical role. We record the submission date, your acknowledgement of this notice, your optional future-contact preference, and recruiting review status.
How we use your information
We use your information to review qualifications, manage applications or professional-network registration, communicate about your submission, and consider relevant opportunities. License and qualification verification may occur during recruitment. We do not use this form to make automated hiring decisions.
Who can access it
Access is limited to authorized Nova Physical Therapy recruiting administrators and service providers that support secure hosting, storage, and recruiting communications. Nova Physical Therapy maintains its own recruiting records. Submitting here does not register you with another organization. We do not sell applicant information or share it for cross-context behavioral advertising.
How long we retain it
Recruiting records are assigned a retention review date four years after submission or a later recorded recruiting action. Records may need to be retained longer to meet legal obligations, preserve relevant employment records, or resolve a dispute. Authorized administrators review retention and deletion requests before removing records.
Your choices and requests
Permission to contact you about additional future opportunities is optional and separate from processing this submission. You can withdraw that permission, correct your details, or request access to or deletion of your recruiting information by emailing info@mynovapt.com. Include your submission reference if available. We may verify your identity before responding. Requests are subject to applicable rights and record-retention obligations, and exercising a privacy right will not affect fair consideration of your application.
What to leave out
Do not upload patient records, Social Security numbers, identity documents, medical information, financial details, or other sensitive material. We request a résumé and professional qualifications for initial recruiting review.
