Neck Pain After a Car Accident: What to Do Next

The crash itself may have seemed minor. Maybe you felt fine at the scene, exchanged information, and drove home. Then the next morning your neck is stiff, turning your head hurts, and you have a headache you can't shake.

Neck pain after a car accident is very common, and most people improve. This article explains why it happens, which symptoms need urgent care, and how physical therapy can fit into recovery once a doctor has checked you out.

The short answer

Neck pain and stiffness after a car accident are often linked to whiplash, a strain of the neck's muscles, ligaments, and joints caused by the head moving quickly back and forth. Symptoms can take hours or even a day or two to appear. Get medical care first, especially if you have severe pain, numbness, weakness, or a bad headache. Once serious injury has been ruled out, staying gently active and following a guided exercise program are the approaches research supports most.

Why does my neck hurt after a car accident?

In a collision, especially a rear-end crash, your body is pushed forward with the seat while your head lags behind and then snaps forward. That quick motion can strain the muscles, ligaments, and small joints of the neck. This is commonly called whiplash, and the range of symptoms that can follow is known as whiplash-associated disorder.

Common symptoms include (Mayo Clinic):

•          Neck pain and stiffness

•          Pain that gets worse when you move your neck

•          Less range of motion when turning or tilting your head

•          Headaches, often starting at the base of the skull

•          Tenderness or pain in the shoulders, upper back, or arms

•          Tiredness

Whiplash isn't the only possible cause of neck pain after a crash, which is why a medical evaluation comes first.

Why can pain show up a day or two later?

Many people feel fine right after an accident and only notice pain the next day. Whiplash symptoms usually develop within days of the injury (Mayo Clinic). Adrenaline at the scene can mask discomfort, and stiffness and soreness often build overnight as irritated tissues settle.

Delayed pain is common and doesn't by itself mean something serious happened. But it's a good reason to get checked even if you felt okay at first.

What symptoms need urgent care?

Call 911 or go to the emergency room after a car accident if you have:

•          Severe neck pain, especially pain right over the bones of the spine

•          Numbness, tingling, or weakness in your arms or legs

•          A severe or worsening headache

•          Trouble speaking, swallowing, or seeing

•          Dizziness, confusion, unusual sleepiness, or memory problems

•          Loss of consciousness at any point, even briefly

•          Loss of bladder or bowel control, or trouble walking or with balance

Even without these signs, see a physician soon after any crash that leaves you with neck pain. A doctor can decide whether you need imaging, which is not needed for everyone. That medical clearance comes before physical therapy.

When should I start physical therapy?

Once a physician has ruled out a fracture or other serious injury, you don't usually need to wait long to start moving. The American Physical Therapy Association's neck pain guideline recommends education that encourages people with whiplash to return to normal activity, combined with exercise (Blanpied et al., JOSPT 2017; JOSPT, 2017).

In general, research favors staying gently active over long periods of rest or wearing a soft collar for weeks. A physical therapist can help you find the right amount of movement for where you are now.

How can physical therapy help after a car accident?

An evaluation may include:

•          How the accident happened and how your symptoms have changed since

•          Neck and upper back motion

•          Strength and endurance of the neck and shoulder muscles

•          A check of sensation, strength, and reflexes in your arms

•          How pain is affecting sleep, work, driving, and daily tasks

•          Screening for signs that need a physician's follow-up

A typical plan may include:

•          Clear education about what's happening and what to expect

•          Gentle range-of-motion exercises, progressed as you improve

•          Strengthening and endurance exercises for the neck, shoulders, and upper back

•          Hands-on treatment, when it helps you move and exercise more comfortably

•          Guidance on returning to driving, work, and exercise

•          A home program you can keep doing on your own

Questions about pain medication, injections, or imaging are best directed to your physician. Questions about insurance, claims, or legal matters are best directed to your insurer or attorney.

How long does recovery take?

It varies. Many people improve over the first few weeks to months. A 2008 review of whiplash studies found that most improvement happened in the first three months after the injury, while a meaningful share of people still had some neck pain a year later (Kamper et al., Pain 2008). Researchers have also found that recovery tends to follow different paths for different people (JOSPT, 2016).

That's one reason early guidance matters. Physical therapy doesn't guarantee a specific result or timeline, but it can help you stay active, track your progress, and spot when something needs a physician's attention.

Do I need a referral for physical therapy in California?

California allows direct access to physical therapy. A physical therapist can evaluate and treat you without a referral for up to 45 calendar days or 12 visits, whichever comes first. After that, a physician or podiatrist needs to sign the plan of care for treatment to continue. After a car accident, though, see a physician first to rule out serious injury. Your insurance plan may also have its own referral rules.

Frequently asked questions

Is it normal for my neck to hurt the day after a car accident?
Yes, delayed neck pain and stiffness are common after a crash. Still, get checked by a physician, especially if pain is severe or you have any of the warning signs above.

Should I wear a neck brace after whiplash?
Only if your physician tells you to. For typical whiplash, research generally favors gentle movement over long-term collar use.

Do I need an X-ray or MRI?
Not always. Your physician decides whether imaging is needed based on how the accident happened and what they find on exam.

Can a minor accident cause whiplash?
Yes. Even low-speed collisions can cause neck symptoms. How you feel matters more than how the car looks.

Should I rest my neck completely?
Usually not for long. A short period of taking it easy is fine, but guidelines encourage returning to normal activity as you're able, with guidance.

Why do I have headaches after the accident?
Headaches starting at the base of the skull are a common whiplash symptom. A severe, sudden, or worsening headache after a crash needs emergency care.

How soon can I start physical therapy?
Once a physician has ruled out serious injury. Many people start within days to a couple of weeks.
