Hand Numbness and Tingling: Is It Carpal Tunnel?

By Nova Physical Therapy, an outpatient physical therapy clinic in North Hollywood, CA | Published October 2026

You wake up at 3 a.m. with your hand numb or tingling, and you shake it out until the feeling comes back. During the day it happens again while you're driving, holding your phone, or typing. Maybe you've started dropping things.

Those symptoms often lead people to search for carpal tunnel syndrome. This article explains what it is, what may make it worse, what research supports, and when hand numbness needs a doctor's evaluation.

The short answer

Numbness, tingling, or pain in the thumb, index, middle, and part of the ring finger, especially at night, may be associated with carpal tunnel syndrome, which happens when the median nerve is squeezed at the wrist. It's common, and symptoms often come and go at first. A night splint that keeps the wrist straight, along with certain exercises, hands-on treatment, and workstation changes, may help mild to moderate symptoms (Erickson et al., JOSPT 2019). Diagnosis, including nerve testing, is made by a physician. In California, you can see a physical therapist without a referral.

Why do my hands go numb at night?

Many people sleep with their wrists bent, tucked under a pillow or curled toward the body. A bent wrist raises the pressure inside the carpal tunnel, a narrow passage at the base of your palm that the median nerve runs through. Hours of that pressure can leave your fingers numb or tingling when you wake up. The NHS notes that carpal tunnel symptoms are usually worse at night (NHS).

Not every case of hand numbness is carpal tunnel. Similar symptoms can come from a nerve in the neck, other nerves in the arm, or medical conditions like diabetes. That's why a proper evaluation matters.

What is carpal tunnel syndrome?

The carpal tunnel is formed by the small bones of the wrist and a tough band of tissue across the front. Nine tendons that bend your fingers and the median nerve all pass through it. When the space gets crowded or swollen, the nerve can be compressed.

Common symptoms include (NHS; JOSPT Perspectives for Patients, 2019):

•          Numbness, tingling, or pins and needles in the thumb, index, middle, and part of the ring finger

•          An ache or pain in the fingers, hand, or arm

•          A weak thumb or trouble gripping, buttoning, or holding small objects

•          Symptoms that start slowly and come and go

The JOSPT patient guide notes that some people (28% to 62%) recover without treatment, while others (32% to 58%) get worse, which is why it's worth getting checked rather than waiting indefinitely.

What makes carpal tunnel worse?

Things that may raise your risk or make symptoms worse include (NHS):

•          Repeated wrist bending or hard gripping, such as with tools, especially vibrating ones

•          Sleeping with your wrists bent

•          Pregnancy, which often causes temporary symptoms

•          Diabetes, arthritis, or a past wrist injury

•          Higher body weight

•          A family history of carpal tunnel syndrome

Simple steps that help many people:

•          Keep your wrists straight while sleeping, often with a night splint

•          Take short breaks from repetitive gripping or wrist bending

•          Set up your keyboard and mouse so your wrists stay close to neutral, and avoid pounding the keys

•          Loosen your grip on phones, steering wheels, and tools when you can

Can physical therapy help carpal tunnel?

For mild to moderate symptoms, the 2019 APTA carpal tunnel guideline and its patient summary support (Erickson et al., JOSPT 2019; JOSPT Perspectives for Patients):

•          A night splint that holds the wrist in a neutral position, for short-term relief

•          Exercises, combined with the splint, for mild to moderate symptoms

•          Hands-on treatment of the neck and arm by a therapist

•          Workstation changes, especially mouse and keyboard setup

The same guide notes that low-level laser, therapeutic ultrasound, iontophoresis, and magnets have not shown consistent benefit.

A physical therapy evaluation may include:

•          When symptoms started and what brings them on

•          Sensation and strength in your hand and fingers

•          Tests to see whether your neck or other nerves may be involved

•          How you use your hands at work, at home, and while sleeping

•          Screening for signs that need a physician's evaluation

Physical therapy doesn't guarantee a specific result. Some people need further medical treatment, and if non-surgical care isn't helping, surgery may be an option. Nerve testing, injections, and surgery decisions are made by your physician or hand surgeon. Physical therapy can also help with recovery after surgery.

At Nova Physical Therapy in North Hollywood, work injury physical therapy is one of our core services, and every plan starts with an evaluation so treatment fits what we find.

When should I see a doctor?

See a physician promptly if you have:

•          Numbness that is constant rather than coming and going

•          Weakness in your hand, or you're frequently dropping things

•          Shrinking of the muscle at the base of your thumb

•          Numbness in both hands, or numbness in your feet as well

•          Symptoms that are getting worse or haven't improved with home care

•          Numbness after a wrist injury or fall

Call 911 if numbness or weakness in your hand or arm comes on suddenly, especially with face drooping, trouble speaking, confusion, or weakness on one side of the body. These can be signs of a stroke.

We screen for these signs during every evaluation and refer you to a physician when findings call for medical assessment.

Do I need a referral for physical therapy in California?

Not to get started. California allows direct access to physical therapy. A physical therapist can evaluate and treat you without a referral for up to 45 calendar days or 12 visits, whichever comes first. After that, a physician or podiatrist needs to sign the plan of care for treatment to continue. Your insurance plan may have its own referral rules, so check your coverage.

Frequently asked questions

Why do my fingers tingle at night?
Many people sleep with their wrists bent, which raises pressure on the median nerve at the wrist. Keeping your wrist straight, often with a night splint, may help. Ongoing or worsening symptoms should be checked.

Does a wrist brace help carpal tunnel?
A brace worn at night that keeps the wrist in a neutral position is supported for short-term relief of mild to moderate symptoms. The NHS notes it may take up to 6 weeks to notice improvement.

Can carpal tunnel go away on its own?
Sometimes. Research suggests some people recover without treatment, while others get worse. Symptoms linked to pregnancy often improve after the baby is born.

Is my numbness from my wrist or my neck?
It can be hard to tell on your own. A nerve in the neck can cause similar symptoms. Your physician and physical therapist can help sort out where it's coming from.

Do I need a nerve test?
Your physician decides whether nerve testing is needed, often when the diagnosis is unclear, symptoms are severe, or surgery is being considered.

Can typing cause carpal tunnel?
Computer work can aggravate symptoms, especially with bent wrists or a hard keystroke. Adjusting your mouse and keyboard setup may ease symptoms.

Do I need a referral to see a physical therapist?
No, not to start. California allows direct access within the limits described above.
